Register to Vote: Please visit https://myvote.wi.gov/en-us/ or complete form EL-131.pdf and mail to Town of Forestville, PO Box 175, Forestville, WI 54213. If you are a qualified elector, U.S. citizen, at least 18 years of age on or before Election Day and have been a Town of Forestville resident for at least 28 consecutive days, you are eligible to register to vote. Wisconsin also has Election Day Registration. Proof of residency is required to register. Photo ID
Wisconsin residents are required to show proof of identification in order to obtain a ballot, both at the polls and by absentee. In most cases, this will be a Wisconsin driver's license or Wisconsin issued identification card. There are several other documents that will also work. Please refer to https://bringit.wi.gov/for further information about Photo ID. Photo ID requirements are different than proof of residence requirements. Your driver's license does not have to have your current address for use as Photo ID. You can obtain a free State ID card for voting purposes through the Wisconsin Division of Motor Vehicles. Application for Absentee Ballots:
You can request an absentee ballot by visiting https://myvote.wi.gov/en-us/. Photo ID must be provided to obtain an absentee ballot.

By mail. Use form EL-121-Wisconsin-Application-for-Absentee-Ballot.pdf or send a written, signed request stating your name, address, mailing address (if different), a photo ID, and the election you are requesting an absentee ballot for to the Town of Forestville, PO Box 175, Forestville, WI 54213. A ballot will be mailed to the address you specify.

By email. Email a request stating your name, address, mailing address (if different), a photo ID, and the election you are requesting an absentee ballot for to clerk@forestvilletown.wi.gov. A ballot will be mailed to the address you specify.

Absentee in Person Voting. You may vote absentee in person beginning the second Tuesday prior to Election Day and ending on the Friday before Election Day. Absentee voting hours are by appointment M-F 5:00 p.m. to 7:00 p.m. Please call 920-639-2431 or email clerk@forestvilletown.wi.gov to schedule a time.

Your ballot must be received by 8:00 p.m. on Election Day in order to be processed and counted.
